Passport Applications

The Webster Groves Public Library serves as a Passport Application Acceptance Facility for the U.S. Department of State. Passport services are by appointment only.

Appointments are hourly any day the Library is open.

Please call us at 314.961.7277 to schedule a time. If you are an adult renewing your existing passport, please see the State Department’s criteria for renewing by mail. If you meet the requirements, you must submit a DS-82 form directly to the State Department, rather than having an application executed at the Library.

The following is a list of items we will need from you in order to process your application.

  • Your completed DS-11 application form.  The State Department’s website will allow you to fill out and print your application, or print a blank application to fill out by hand. Blank applications are also available at the Reference Desk and many post offices. Forms filled out by hand must use black ink only.
  • One passport photo.  Because of the specific requirements for the photo, it is best to have it taken somewhere that specifically performs that service.  We recommend About Time & American Passport (7515 Big Bend Blvd. in Webster Groves), but most CVS, Costco, FedEx, UPS Store, Wal-Mart, and Walgreen’s locations also have passport photo services available.
  • Proof of US citizenship.  In most cases, this will be an original, certified copy of your birth certificate or a previous US passport.  Other examples include a naturalization certificate or a Consular Report of Birth Abroad.
  • Valid government-issued ID.  A current Missouri driver’s license or US passport. For other forms of ID, such as an expired or out-of-state driver’s license or non-driver’s ID, you may need to bring supplementary ID. Please see the State Department’s website for details.
  • For all child passports (under age 16), the child must be present and accompanied by both parents/guardians. For exceptions, see the State Department’s website (scroll to “7. Show Parental Consent”). Applicants age 16 or 17 must demonstrate parental awareness of their application, preferably by being accompanied by at least one parent who will cosign the application.
  • Individual checks or money orders payable to the US Department of State for each application being submitted.  These fees cannot be paid by cash or credit card.  Current application fees are as follows :
    • $130 for a passport book (ages 16 and over)
    • $100 for a passport book (ages 15 and under)
    • $30 for a passport card (ages 16 and over)
    • $15 for a passport card (ages 15 and under)
    • $60 for optional expedited service.  (Check the State Department’s website for current routine and expedited processing times.)
    • $19.53 for optional 1 – 2 day delivery of your passport from the State Department.
  • Separate $35 execution fee per application paid to Webster Groves Public Library.  This payment can be by cash, credit card, or check.

We accept applications for either Routine or Expedited processing as defined by the Department of State. We cannot accept passport applications for Urgent or Emergency Travel, or those that will delivered to the Department of State for processing by a third-party courier service (also know as “hand-carry” applications).
